5. Is he or she physically or verbally abusive?
6. Is he or she becoming more forgetful?
7. Have there been recent falls?
8. Is the individual having trouble sleeping?
9. Has there been a recent weight loss?
10. Has he or she lost interest in eating?
11. Is his or her hearing or vision loss affecting the ability to function?
12. If he or she smokes, are there burn marks around?
13. Is the individual able to do errands alone?
14. Is clothing being changed regularly? Are there old food stains on clothing?
15. Is the individual content to just sit in a chair?
16. Are there scorch marks on pot holders or dish towels?
17. Is there less participation in conversation?
18. Are there signs of burnt pans on the stove or stored in cupboards?
19. Is routine house cleaning not being done?
20. Is there an odor in the home?
21. Have social activities diminished or stopped altogether?
22. Is their car dirty and has dents and dings?
